Emergency medical card in Japanese

Fill in the ten fields an ambulance crew or a hospital reception desk asks for first, and this prints them on a wallet card with a Japanese label in front of every value — 血液型, 持病, 服用中の薬, 緊急連絡先 — so somebody who reads no English can still act on it.

Nothing is uploaded — the card is built in your browser

This tool only prints what you type. It gives no medical advice, makes no diagnosis and guarantees no translation of anything you write yourself.

What goes on the card, and why those ten fields

Name, nationality, date of birth, blood type, drug allergies, existing conditions, current medication, emergency contact, travel insurance and embassy. Those are the ten a Japanese emergency department works through, and the four that change treatment fastest are the drug allergies, the conditions, the medication and the blood type. Each value is printed after its Japanese label, so 服用中の薬: levothyroxine 50ug daily is readable to staff even though the value itself is in English.

The Japanese the card opens with

The front leads with 日本語が話せません。救急のときはこのカードを見せてください。 — I do not speak Japanese, please show this card in an emergency — and closes with 救急車は119番です。, a reminder that the ambulance number in Japan is 119, not 911 or 112. Calling 119 is free from any phone, including a payphone, and the ambulance itself is free; you pay the hospital, not the ride.

Why a printed card beats a phone

An unconscious traveller cannot unlock a phone, and a phone that has run flat carries nothing at all. A card in a wallet, a passport holder and a day bag is readable by anyone who finds you. Print the ten-up A4 sheet once and you have copies for every bag plus one to leave with whoever you are travelling with.

Health data that never leaves the browser

Nothing is uploaded. Symptoms, medicines and contacts are typed into your own browser tab, and the PDF is assembled there by JavaScript, so health information never reaches a server. There is no account, no sync and no analytics on what you type. That matters more here than anywhere else on the site: this card holds your medication list and your existing conditions.

Printing and sizes

The default is 91 by 55 millimetres, ten to an A4 or US Letter sheet with cut lines, as a one-page PDF. Choose A6 for four larger cards per sheet, or A5 for a single sheet with big type if you want it readable at arm's length. Ticking the back side adds a second page with the English text, mirrored so it registers when the paper is flipped on the long edge.

What this card does not do

This page gives no medical advice and makes no diagnosis. The tool prints exactly what you tick or type, nothing is checked or professionally translated on your behalf, and anything you write yourself is printed word for word.

Frequently asked questions

What is the emergency number in Japan?

Dial 119 for an ambulance or a fire, and 110 for the police. Both are free from any phone including payphones, and the operator will usually find someone who speaks some English, though it can take a moment. The ambulance ride itself costs nothing.

Will a Japanese hospital accept my travel insurance?

Many hospitals ask you to pay in full on the day and claim the money back afterwards, so carry a card that works and keep every receipt. Printing the insurer name and policy number on this card means the reception desk can at least see who to contact rather than starting from nothing.

Should I write my medication in English or Japanese?

English is fine, and the generic name is far better than the brand name — brands differ between countries while the generic name is what a Japanese doctor recognises. The card prints the Japanese label 服用中の薬 in front of whatever you type, so staff know what they are reading even if the value is Latin script.

Is my medical information uploaded?

Nothing is uploaded. Symptoms, medicines and contacts are typed into your own browser tab, and the PDF is assembled there by JavaScript, so health information never reaches a server.
